In the mid-1990s, Bristol, England, saw very high mortality for surgery in congenital heart disease, followed by contentious public inquiry. One of the important findings of a subsequent study was that the journey from the operating room to the intensive care unit (ICU) was high risk.
My perspective: Lots of case studies about observing Formula 1 teams, but I liked this one in that it identified the value of both process mapping and stepping back and taking a wider perspective, especially around risk, relatively simple changes led to significant reduction in errors!

My perspective: Radical example how Covid-19 will force change and new financial disciplines on many organisations - I was interested in all the different income streams that a club can have, but when you are spending 65-70% of revenue on player salaries, you are certainly going to struggle bringing about change, the you combine ego, fans, and status along with Covid, you have an uphill battle.
